Community Health Hubs

Delivering equitable health to local communities through partnership and collaborations

CHMRC implements and hosts pop-up, collaborative, and permanent health hubs in partnership with public health and local community based organizations to provide essential services. These services include vaccinations, screenings, testing, activities, support structures, learning opportunities, and referrals to local community services.

Pop-Up Health Hub Model

Our pop-up health hub model, usually set up under a popup tent or temporary structure, will offer a range of services, such as health screenings, vaccinations, counseling, and education on various health topics. These hubs can be set up in a variety of locations, such as community centers, public spaces, or even mobile units that travel to different locations. Our pop-ups also help to reach populations that may not regularly seek out healthcare services or may face other barriers to accessing care, such as language barriers or transportation issues.

Collaborative Health Hub Model

In our collaborative health hub model CHMRC will brings together and coordinates various healthcare providers and community organizations to work together in a coordinated manner to improve health outcomes for your community. Together, these providers and organizations work together to share information, coordinate care, and address the social determinants of health that can impact a person’s well-being.

Permanent Health Hub Model

Our permanent health hub model, often deployed in partnership with Public Health, are usually located in a permanent facility in an easily accessible location convenient to members of the community.
